Summary:
Under the supervision of the Unit Manager, Assistant Director of Nursing and the Director of Nursing, the Licensed Practical Nurse (LPN) assumes responsibility and accountability for a group of residents/patients for a shift of duty.
Nursing care is provided through assessment, implementations, and evaluation of the plan of care.
The LPN adheres to the standards of care for the area, manages the environment to maintain resident/patients' safety, and supervises the resident/patient care activity performance by nursing assistants and helpers.
DUTIES/
Responsibilities:
o Provides routine care for patients during respective shift.
o Provides required patient assessments/observations and interventions.
o Plans and manages patient care according to each patient's needs.
o Interviews patients and records their medical history and physician conditions.
o Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration.
o Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state, and local standards, guidelines, and regulations o Gives/receives the nursing report upon reporting in and ending shift duty hours.
o Ensures assigned work area (i.
e.
nurse station, med.
Cart, medication room, etc.
), resident/patient care rooms and treatment areas are maintained in a clean and sanitary manner.
o Monitors supplies and orders as needed.
o Attends regularly conducted staff meetings as scheduled by the Director of Nursing/Unit Manager.
o Participates regularly in continuing education in-service programs.
o Reviews care plans, at least daily, to ensure appropriate care.
o Appraises the DON / Unit Manager of resident /patient status changes in condition.
o Administers catheter care as required (may include catheter insertion).
o Administers IV therapy as ordered as license allows.
o Provides respiratory care, e.
g.
oxygen, nebulizer treatments.
o Assists the physician in therapeutic measures, as needed.
o Participates in care plan development and attends resident/patient care conferences.
o Receives and transcribes physician's orders to resident's/patient's charts, Kardex/Care Medication Administration Record (MAR), treatment/care plans, as required.
o Prepares and administers medications and performs treatments as ordered by the Physician.
o Charts nurses notes in an informative and descriptive manner that reflects the care provided as well as the resident's/patient's response to the care.
o Documents all pertinent data as required by Medicare, Medicaid, Villa standards and policies.
o Completes the required forms upon admission, transfer, and/or discharge.
o Informs physician of resident/patient change of condition.
o Counts all controlled drugs with other nurse at shift change and notifies the DON / Unit Manager of all drug and narcotic discrepancies noted on your shift.
o Keeps medication room and medication cart clean and orderly and locked according to regulations.
Responsible for security of keys.
o Ensures that crash cart is appropriately stocked and in good repair.
o Ensures the resident environment is as free from accidents and hazards as is possible.
Immediately correct/address potential safety risks as prudent and warranted.
o Directs the day-to-day functions of the certified nursing assistants in accordance with current rules, regulations and guidelines that govern long-term care facilities.
o Closely monitors and supervises all facility residents per facility policies and as warranted by good nursing judgment.
o Coordinates overall resident care issues per nursing license, facility policy and as warranted to meet resident needs.
o Makes written and oral reports/recommendations concerning the activities of shift as required.
o Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care.
o Communicate and reports information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines.
o Ensures that all aspects of resident care plans are implemented and maintained.
Makes recommendations for care plan changes to the IDT per professional standards.
o Ensures that all personnel providing care to residents are providing such care in accordance with the resident's care plan and wishes.
o Admits, transfers and discharges residents as necessary to meet the resident's individual needs and maintain the overall safety of other residents, staff and visitors.
o Ensures compliance with infection control standards.
Immediately correct/address identified instances of non-compliance.
o Participates in the Interdisciplinary Plan of Care (IPOC) process as assigned.
o Attends and participates in in-service classes, performance improvement (PI) committees, and other meetings as assigned.

o Reports resident concerns, complaints, comments to appropriate Administrative personnel.
o Maintains confidentiality of all resident information.
o Safety o Knows and follows Villa Center Rules.
o Demonstrates proper use of equipment.
Reports equipment needs or repairs.
o Follows facility smoking policy.
o Reports and follows appropriate protocol to report any accidents/incidents of residents, staff or visitors to the appropriate Administrative personnel.
o Reports any hazardous conditions/equipment.
o Understands and follows any Disaster Plans.
o Understands and follows infection control standards, policies and procedures.
o HIPAA o Follows and adheres to Villa's policies and procedures regarding HIPAA compliance.
o Ensures integrity and confidentiality of all patient financial data.
o Reports all suspected violations of company's HIPAA policies and procedures to appropriate Administrative personnel.
o Responsible, attentive, knowledgeable and provides high quality care to patients.
o Must be able to adapt to a flexible work schedule, including overtime, holidays, nightshifts, and weekends.
o Possess special interest in, and a positive attitude about, working with short and long-term care residents and the elderly.
o Must be willing to participate in all-hands on dining.
o Ability to read, writes, speak, and understand English.
o Meets all health requirements, as required by law.
o Demonstrate basic computer knowledge and ability with an aptitude to learn company applications.
Microsoft Office version 1997, 2000 or XP, MS Word, MS Access, MS Excel, E-mail, Outlook, Internet Explorer.
o Perform other duties as assigned and consistent with level of preparation and experience.
EDUCATION and
Experience:
o Nursing Degree or diploma from an accredited College or University.
o Possess a current license to practice in the State as a Licensed Practical Nurse.
o 1 year of experience preferred.
o Long-Term care experience preferred.
ENVIRONMENTAL AND PHYSICAL REQUIREMENTS:
o Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
o Limited potential for exposure to environmental hazards.
