MDS Coordinator - RN RN will work with residents, their family and the facility staff to coordinate care received by all disciplines throughout the course of their stay.
The coordinator will complete the MDS utilizing current RAI manual guidelines and oversee accurate and timely completion of all Medicare residents' MDS assessments.
The coordinator will act as resource for the RN's, LTC nurses and all others requiring MDS support.
The MDS Coordinator will work in collaboration with the rehab director and nurse manager in an effort to provide excellence in the quality of care and customer service delivered by Lake Orion Nursing & Rehabilitation Center.

Specific tasks may include:
Act as team leader for the Interdisciplinary Medicare Meeting Create a schedule for Medicare residents PPS/OBRA required assessments and oversee its completion Completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment Coordinate the completion of CAA's for each triggered item per current regulations Complete the Care Plan that meets current regulatory standards for each resident Interact with residents and families during the care planning process to keep them informed of care plan goals Complete all Discharge and Re-entry assessments as required by regulation Minimum
Qualifications:
Graduate from an accredited school of Nursing Registered Nurse Must possess current Licensure in State of Michigan Minimum of two years' experience of charge/progressive/leadership experience Must demonstrate competence in assessment skills and computer literacy.
Possess good organizational ability Possess good oral and written communication skills Preferred:
